We can have an agreement, a plan and a growing evidence file—and still be unclear about who may take the next decision.
That is the space I want this publication to examine.
The Governance Spine gives us four connected questions: Fracture → Signal → Spine → Assurance. Where has responsibility become unclear? What is the record telling us? Who reviews and who decides? Can someone else reconstruct the basis of the decision later?
I am interested in the distinction between something being visible and something being proven; between a document being submitted and a finding being accepted; between a relationship being agreed and the next activity being authorised.
Those distinctions need not make work more complicated. They can help us name the actual question, the person responsible for it and the conditions that still need to be resolved.
This blog is a place for practical explanations and reflection. It does not take the place of an institution’s own judgement, and it should not claim that a framework can guarantee delivery.
The aim is a clearer, more accountable basis for action—one decision at a time.
