What connects the four parts?

A gap is identified. The concern enters the record. Evidence and responsibility are connected to a named review and decision route. The resulting record preserves what was considered, what remains uncertain and who acted.

New evidence may reopen an earlier question. The route is not a one-way claim that a programme has progressed from failure to approval.

One practical example

A contract milestone has no named reviewer. That is a Fracture. Recording the missing role and its source creates a Signal. Assigning review responsibility through the appropriate authority connects the Spine. Preserving the decision, version and remaining conditions supports the Assurance question: can this be reconstructed?
