An evidence record may be valid and still have no clear route to a decision. Someone must identify the question it supports, the review it requires and the institution authorised to act.
Make the intended use explicit
Record the claim, the supporting source and the limits of the evidence. Identify the receiving reviewer and the criteria relevant to that specific use.
Keep the finding separate from the decision. A reviewer can assess a claim without holding authority to approve the activity that follows.
The purpose of a decision path is not to make evidence automatically actionable. It is to make the conditions of its use visible and accountable.
